Dear parents,
The government school medical team will be coming to Stella's School during the second term for their annual health screening. All year one students have been given a new school health record to take home. If you could please help us to complete the first page details and return the record with your child's small clinic card (yellow/pink) to the class teacher for the nurses reference. The small clinic card will be returned to you as soon as the nurses have completed their work. For students that have been transferred from another school we require your child's medical records from their previous school.
