H1N1 INJECTION
INDOOR NATIONAL STADIUM
INDOOR NATIONAL STADIUM
In collaboration with the Ministry of Health efforts to prevent the spread of H1N1, the Ministry Of Health has kindly set aside two dates for students of Stella's School who have not yet received the H1N1 injection to do so.
The dates are as follows:-
MONDAY, 5TH APRIL 2010
Time: 8.00 am to 12.00 noon - 2.00 pm to 4.00 pm
TUESDAY, 6TH APRIL 2010
Time: 8.00 am to 12.00 noon
VENUE: INDOOR NATIONAL STADIUM BERAKAS
For those parents who wish their child to be injected, please bring our child (DIRECTLY) to the indoor National stadium together with the form attached on one of these set dates.
(Please inform your child's class teacher that your child will be absent on that particular day.)
For those parents whose child has already received the injection or do not wish their child to receive the injection, please fill out the form and return it back to the class Teacher as soon as possible.
